Leave of Absence Forms for Postgrads and Postdocs: 

To request a leave, submit the required forms with signatures to the Postdoctoral Affairs Office before starting your leave request on Workday. For guidance, refer to the Workday Guide and the Leave Policy for Postgraduate and Postdoctoral Appointees on the Postdoc Leave Policy page.

Yale School of Medicine Forms 

• FY 26 Laboratory Associate Application: Application must be submitted for approval to postdoc.affairs@yale.edu before creating a req in Workday.. Laboratory associates and visiting fellows receive compensation aligned with Yale’s minimum salary standards for postgraduate trainees.
• Laboratory Associate New Hire Letter 
• Sample Invitation Letter: Please contact the Postdoc Affairs office with any questions regarding appointment title type and eligibility for Postgraduates, Visiting Fellows, and Lab Associates prior to sending an invitation letter. This will avoid delays or miscommunication during the appointment process. 

Yale Central Campus & School of Public Health Forms 

• FY26 Visiting Fellow/Lab Associate Application: The application must be submitted for approval to postdoc.affairs@yale.edu before creating a req in Workday. Laboratory associates and visiting fellows receive compensation aligned with Yale’s minimum salary standards for postgraduate trainees. 
• FY27 Visiting Fellow/Lab Associate Application:  The application must be submitted for approval to postdoc.affairs@yale.edu before creating a req in Workday. Laboratory associates and visiting fellows receive compensation aligned with Yale’s minimum salary standards for postgraduate trainees. 
• Laboratory Associate/Visiting Fellow New Hire Letter
• FAS Postdoctoral/Postgraduate Appointee Invitation Letter: All appointing units or PIs in the FAS who wish to appoint postdoctoral or postgraduate scholars must send formal invitation letters to candidates, laying out in detail the terms of their appointments. This is an FAS-specific requirement. Please contact the FAS Dean’s Office (fas.dean@yale.edu) for questions regarding this template.

Templates & Sample Letters 

• CV Template: Candidates are encouraged to utilize the provided CV template when submitting their applications.  
• Postgraduate Position Description Template: Academic units should email their open position announcements to our office for posting. Please note that postgraduate positions must include section headings for Education, Mentoring, and Training. Use this template when preparing these announcements. 
• Reappointment Letter Template for less than 12 months: Required for all postdocs who are reappointed for less than 12 months.
